Job Description
Provide direct support to a licensed clinical social worker and Day Treatment Program by assisting with client intake, maintaining case files, scheduling appointments, conducting initial assessments, linking clients to community resources, and facilitating communication with other professionals, all under the supervision of the Clinical Director to ensure clients receive appropriate care and support.
Key responsibilities may include:
· Client intake and screening:
Gathering basic information from new clients, scheduling initial appointments, and completing intake paperwork.
· Case management:
Maintaining client records, updating case notes, tracking progress on treatment plans, and coordinating with other service providers.
· Resource linkage:
Assisting clients in accessing community services like housing assistance, food banks, healthcare, and transportation.
· Administrative tasks:
Scheduling appointments, managing phone calls, and preparing reports.

Participating in community events to promote awareness of social services.
