Overview: Sentara Healthcare is seeking a Transplant Coordinator for our Kidney/Pancreas Transplant team at Sentara Norfolk General Hospital. Shift: Days M-F
Outpatient clinic is open Monday - Friday between 8am and 4:30pm. Transplant Coordinator will have some scheduled call and have weekend call about every 11th week.
Position Highlights: Innovation: 525-bed Level 1 trauma center and tertiary care facility that was first in the region to perform kidney, heart, and pancreas transplants.
Market: Sentara Transplant Center has been around for nearly 40 years and is the only transplant center in Hampton Roads Region. We have performed more than 2,200 transplants.
Nursing Excellence: We were the first hospital in the region to become a Magnet® recognized hospital.
What you will do: - Coordinate care with post-transplant patients.
- Facilitates clinic flow, transplant evaluation, and patient care management.
- Provide weekly education sessions to patients and support member. Arranges appointments with surgeons, social worker, financial coordinator, pharmacy and dietary.
- Transplant Coordinator admits patient, provides support to patient/caregiver during the waiting period, and discharges transplant patient with instructions for going home.
- Transplant Coordinator will manage a case load of patients, remain patient's point of contact through the transplant process, recovery and at home.
Requirments: - BSN required
- should have 2+ years of acute care experience, good medical - renal knowledge, and understands chronic disease.
- BLS required.
- Certified Clinical Transplant Coord (CCTC) required within three (3) years of hire.

Sentara Norfolk General Hospital , located in Norfolk, VA, is a 525-bed tertiary care facility that is home to the only Level I Adult Trauma Center and burn trauma unit in Hampton Roads, and also serves as the primary teaching hospital for Eastern Virginia Medical School.
