Key Responsibilities

• Perform comprehensive patient evaluations including history intake, systems review, and administration of specific tests and measures to collect clinical data.
• Establish individualized plans of care and select treatment techniques that address identified impairments in accordance with physician orders.
• Conduct ongoing reassessment of patients and their response to treatment, initiating and managing discharge planning from the first visit through completion of care.
• Supervise and delegate to ancillary staff and students in accordance with state laws and department policy.
• Complete accurate, legible documentation within established timeframes and in compliance with accreditation and insurance requirements.
• Communicate effectively with patients, families, caregivers, and members of the healthcare team to promote maximum benefit of care.

Required Qualifications

• Minimum of 2 years of physical therapy experience in a Long Term Care (LTC) setting.
• Active Texas state license or compact license recognized in Texas.
• Ability to accurately assess, differentially diagnose, and identify deficits in patient mobility and functional activity.
• Must practice within the scope defined by the state practice act.

Preferred Experience / Skills

• Experience with quality improvement activities within a clinical setting.
• Demonstrated ability to assess and address educational needs of patients, families, and caregivers.
